American Heart Association Senior Manager Quality Systems Improvement Programs, Clinical Practice - National Hypertension Control Initiative in Dallas, Texas
The National Hypertension Initiative is funded by the Department of Health and Human Services (DHHS) Office of the Assistant Secretary of Health, with funding from the Office of Minority Health and the Health Resources and Services Administration. The National Hypertension Initiative will improve COVID-related health outcomes for highly impacted racial and ethnic minorities by addressing hypertension as a key risk factor. The American Heart Association (AHA) will provide technical assistance to Health Resources and Services Administration (HRSA)-funded health centers to increase provider and clinician engagement in implementing evidence-based practices and will also include a community integration component, as well as a national targeted media and public education campaign.
The American Heart Association (AHA) has an excellent opportunity for a Senior Manager, Quality Systems Improvement Programs, Clinical Practice based out of our National Center office located in Dallas, TX (remote/home based is acceptable) .
This is a full time, benefits eligible, grant funded opportunity. Current funding will expire 12/2023.
This player/coach will be responsible for the day-to-day quality improvement functions, predominantly in the outpatient setting, of the DHHS-funded National Hypertension Initiative, jointly funded by the DHHS Office of Minority Health (OMH) and Health Resources and Services Administration. The vision of the Initiative is to address health disparities in hypertension among racial and ethnic groups nationwide through a multi-prong strategy targeted to healthcare centers and clinicians, communities, and the public.
Responsible for acting as a clinical care consultant on quality improvement for treatment of hypertension through outpatient programming. Responsible for developing and executing operational plans leading to quality improvement in hypertension control and treatment with a focus on clinical response to underserved populations. Responsible for leading field program managers in engaging with designated Community Health Centers in the evidence-based use of the Measure Accurately, Act Rapidly, Partner with Patients (MAP) framework with a focus on Self-Measured Blood Pressure (SMBP) to improve blood pressure control in vulnerable populations served. Responsible for supporting systematic assessments and improvements of clinical practice habits, operational processes related to device distribution, data flow, and office workflow. Responsible for collaborating with public health and community impact colleagues to engage in clinic-community interaction.
Develop, communicate, and execute quality improvement operational plan for the National Hypertension Control Initiative in the assigned region. Lead a team of program managers to engage HRSA-funded Community Health Center clinic care sites in cohort groups and individually to assess current practice and identify areas for improvement based upon practice data, provider/care team knowledge and skills, and clinical workflow processes and resources.
Provide evidence-based educational resources, practice change facilitation, and quality improvement activities needed to adopt self-measured blood pressure practice and improve blood pressure control in clinic’s patient population. Cultivate relationships with clinical leads at participating sites. Provide training through webinars, regional quality education events, and one on one facilitation to improve care.
Lead and direct the onboarding of clinics and other appropriate healthcare sites into the initiative. Establish and oversee achievement of project goals and objectives. Communicate through virtual meetings and collaboratives. Provide resources and training for technical aspects of grant activities related to blood pressure devices, connectivity between devices and electronic health records, and data reporting and analysis of provider and clinic, and health system population levels.
Collaborate with public health and community impact colleagues to establish Community Health Center to community linkages that help to address referrals, continuity of care, and social determinants of health.
Collaborate and across NHCI project team to help evaluate and implement objectives and assist in the expansion of the program as needed. Support evaluation and tracking of grant deliverables and contractual obligations. Develop and deliver account plans and management strategies with program operations team.
Clinical knowledge/experience working with clinical practice guidelines, preferably with blood pressure and/or chronic condition including aspects of blood pressure measurement, medication management, and patient engagement and adherence strategies
Experience working in healthcare delivery with operational knowledge of primary care practice environments and ambulatory clinic workflow
Experience/training in practice facilitation
Experience leading a team
Quality improvement knowledge/skills regarding various methodologies (e.g., Lean, Six Sigma, PDSA, TPS) and comfort analyzing data for performance improvement opportunities
Comprehensive knowledge of the Community Health Center setting with culturally and linguistically appropriate services (CLAS)
Basic knowledge and understanding of healthcare information technology such as e-medical records, healthcare IT or related systems or database functions
Ability to analyze and interpret technical requirements to improve data sharing and integration
Project management skills and the ability to influence performance without direct line accountability
Educational skills to structure group training for adult learners and team-based caregivers
Strong interpersonal and relationship building skills
Excellent oral and written communication
Ability to work in a team environment and interact with all levels of American Heart Association staff, volunteers and the public
Comprehensive Microsoft Office suite skills including Word, Excel, PowerPoint and Outlook
Master’s Degree in clinical expertise
Knowledge of assigned health concern or disease such as CVD risk factors
