American Heart Association Community Health Manager - Temporary in Philadelphia, Pennsylvania

The American Heart Association (AHA) has an excellent opportunity for a Community Health Manager (Part Time) , based in Delaware. The primary focus for this position is related to implementation of blood pressure focused initiatives within clinics that support the AHA’s Blood Pressure Control strategic plan, as well as working with community and faith-based organizations to implement the evidenced-based initiatives. This is a grant-funded, temporary, part time ( 20/hours per week ) implementer position anticipated for approximately 6 to 7 months.

The Community Health Manager will develop and implement a robust market plan to increase clinic engagement programs focusing on our national Target: Blood Pressure (Target: BP) Recognition Program , as well as community-based and self-monitoring initiatives. Target: BP is a joint effort between the AHA and the American Medical Association to help participating practices to achieve high levels of blood pressure control using the latest AHA evidenced-based guidelines, with the goal of motivating hospitals, medical practices, practitioners, and health services organizations to prioritize blood pressure control and improve patient outcomes. Healthcare providers will be recruited to join the program and work within their systems to increase their control rates for High Blood Pressure patients.

Essential Job Duties:

  • Function as the primary internal and external contact and resource for the program.

  • Promote and expand positive community relations through active outreach.

  • Work with the staff of physician’s practices and clinics to enroll online for Target: BP Recognition and upload data into the recognition portal.

  • Review data collected from various sources, collaborate with clinical teams on quality and process improvement recommendations to improve performance.

  • Identify, cultivate, and manage relationships with clinic accounts, and other key healthcare stakeholder activities with regards to blood pressure control processes.

  • Work with clinical teams (Physicians, Nurses, Quality Improvement, etc.) on quality and process improvement initiatives, specifically with regards to blood pressure.

  • Where feasible, host lunch and learn sessions with lead nurse, and/or medical directors on how to sign-up for the Target BP Recognition, and how to upload data in the online AMA Target:BP Recognition portal.

  • Engage and educate the community with regards to blood pressure resources (coordinate and facilitate market workshops, training, etc. with community, and faith-based organizations) and other health-related initiatives as identified.

  • Build meaningful volunteer partnerships to advance the mission of the AHA.

  • Manage the Hypertension Network and recruit additional members.

  • Regularly evaluate and report on progress towards stated objectives and provide support for achievement of the grant funding deliverables. Facilitate the completion of pre- and post-evaluations.


Qualified candidates are highly motivated to further the mission of the American Heart Association and possess the following skills, education, and experience.

  • Bachelor’s degree preferred or equivalent experience in health education, nursing, health administration, public health, health science, business or related field or equivalent experience.

  • Exceptional oral and written communications skills.

  • Ability to productively participate in a multi-disciplinary team environment and manage large projects ensuring deadline compliance with minimal supervision.

  • Highly effective organization, multi-tasking, communication, negotiation, and interpersonal skills.

  • Ability to conduct meetings with internal staff and volunteers including interacting with external partners/sponsors representing AHA to the public, as necessary.

  • Ability to work in a fast-paced environment and collaborate daily with staff from various teams.

  • Ability to work, as needed, outside standard business hours and travel within territory served.

Here are some of the preferred skills we are looking for:

  • Knowledge of health care systems, clinical settings, public health, voluntary health organizations, social determinants of health, and nonprofit organizations.

  • Experience in volunteer recruitment and management to drive health impact goals.

  • Experience with data analysis a plus.
